Objective:
Obesity is associated with adverse effects on brain health, including increased risk for neurodegenerative diseases. Changes in cerebral metabolism may underlie or precede structural and functional brain changes. While bariatric surgery is known to be effective in inducing weight loss and improving obesity-related medical comorbidities, few studies have examined whether it may be able to improve brain metabolism. In the present study, we examined change in cerebral metabolite concentrations in participants with obesity who underwent bariatric surgery.
Participants and Methods:35 patients with obesity (BMI > 35 kg/m2) were recruited from a bariatric surgery candidate nutrition class. They completed single voxel 1H-proton magnetic resonance spectroscopy at baseline (pre-surgery) and within one year post-surgery. Spectra were obtained from a large medial frontal brain region. Tissue-corrected absolute concentrations for metabolites including choline-containing compounds (Cho), myo-inositol (mI), N-acetylaspartate (NAA), creatine (Cr), and glutamate and glutamine (Glx) were determined using Osprey. Paired t-tests were used to examine within-subject change in metabolite concentrations, and correlations were used to relate these changes to other health-related outcomes, including weight loss and glycemic control.
Results:Bariatric surgery was associated with a reduction in cerebral Cho (f[34j = -3.79, p < 0.001, d = -0.64) and mI (f[34] = -2.81, p < 0.01, d = -0.47) concentrations. There were no significant changes in NAA, Glx, or Cr concentrations. Reductions in Cho were associated with greater weight loss (r = 0.40, p < 0.05), and reductions in mI were associated with greater reductions in HbA1c (r = 0.44, p < 0.05).
Conclusions:Participants who underwent bariatric surgery exhibited reductions in cerebral Cho and mI concentrations, which were associated with improvements in weight loss and glycemic control. Given that elevated levels of Cho and mI have been implicated in neuroinflammation, reduction in these metabolites after bariatric surgery may reflect amelioration of obesity-related neuroinflammatory processes. As such, our results provide evidence that bariatric surgery may improve brain health and metabolism in individuals with obesity.

OBJECTIVES/GOALS: The current proposal seeks to investigate the effect of early life antibiotic use in the development of functional gastrointestinal (GI) disorders. We propose that infants exposed to antibiotics will present with gut microbial dysbiosis, changes in fecal bile acid concentrations and develop more GI symptoms compared to unexposed children. METHODS/STUDY POPULATION: We analyzed fecal samples from 174 subjects at 12 months of age, of whom 52 were exposed to antibiotics in their first year of life. Of these, 33 subjects were sampled again at 24 months of age. DNA from 200mg of frozen stool (−80C) was isolated with the Qiagen DNeasy PowerSoil kit. Shotgun libraries were generated using the NexteraXT kit and sequenced on the Illumina HiSeq 2500 using 2x125 bp chemistry. Sequence data were analyzed using the Sunbeam metagenomics pipeline. The abundance of bacteria was estimated using Kraken version 2.0.8. Fecal bile acids will be quantified by liquid chromatography–mass spectrometry (LC-MS). RESULTS/ANTICIPATED RESULTS: Overall bacterial community composition at 12 or 24 months was not associated with antibiotic exposure (PERMANOVA test, Bray-Curtis distance). An increase in Enterobacteriaceae, in particular Escherichia coli, is a signature of antibiotic-induced dysbiosis, but also of early infant gut. Children with antibiotic exposure had slightly higher abundance of Escherichia coli compared to those with no exposure (p = 0.03). At 24 months, the abundance of Bacteroides caccae, a commensal gut species, was decreased for children exposed to antibiotics in the first year of life (fdr = 0.02). We will perform further analysis of bile acid modifying bacteria, fecal bile acid concentrations and correlate to GI symptoms. DISCUSSION/SIGNIFICANCE OF IMPACT: Our findings suggest a significant but nuanced impact of early life antibiotic use on the composition of the gut microbiota. The association of antibiotic exposure with B. caccae and E. coli warrant further attention in the context of the rapidly developing early-life microbiome. National security is one of many fields where experts make vague probability assessments when evaluating high-stakes decisions. This practice has always been controversial, and it is often justified on the grounds that making probability assessments too precise could bias analysts or decision makers. Yet these claims have rarely been submitted to rigorous testing. In this paper, we specify behavioral concerns about probabilistic precision into falsifiable hypotheses which we evaluate through survey experiments involving national security professionals. Contrary to conventional wisdom, we find that decision makers responding to quantitative probability assessments are less willing to support risky actions and more receptive to gathering additional information. Yet we also find that when respondents estimate probabilities themselves, quantification magnifies overconfidence, particularly among low-performing assessors. These results hone wide-ranging concerns about probabilistic precision into a specific and previously undocumented bias that training may be able to correct.

The star has to go on radiating and radiating and contracting and contracting until, I suppose, it gets down to a few km. radius, when gravity becomes strong enough to hold in the radiation, and the star can at last find peace. Dr. Chandrasekhar had got this result before, but he has rubbed it in in his latest paper; and, when discussing it with him, I felt driven to the conclusion that this was almost a reductio ad absurdum of the relativistic degeneracy formula.

General relativistic astrophysics encompasses a broader arena, and separate chapters or parts of chapters in this volume are devoted to cosmology, gravitational waves, the inspiral and merger of compact binaries, and black-hole stability.
Relativistic astrophysics began in 1916 on the Russian front, where Karl Schwarzschild wrote two papers, one reporting the solution to the Einstein equation for an incompressible spherical star, the other presenting the celebrated vacuum Schwarzschild spacetime. Schwarzschild was dead within the year, and for the next 47 years his solutions had a twilight existence. In no known stars did general relativity play a significant role, and only a handful of papers in astronomy or astrophysics mentioned the work.
Although sparsely distributed, the exceptions to this neglect were remarkable. In 1931, shortly before Chadwick's discovery of the neutron and shortly after the first paper by Chandrasekhar [2] (following approximate computations by Anderson [3] and Stoner [4] on an upper mass limit of white dwarfs, Landau [5] submitted a paper that independently argued that there was an upper limit on the mass of a collection of degenerate fermions and speculated on the existence of stars with cores of nuclear density.

Cardiac troponin I after cardiopulmonary bypass in infants in comparison with older children

Objective
At the present time, there is a trend towards performing open heart surgery at a younger age. Myocardium of infants has been thought to be more vulnerable to cardiopulmonary bypass in comparison with adults. For this study, we evaluated the degree of myocardial injury by measurement of cardiac troponin levels in infants in comparison with older children for similar surgeries.
MethodsSerum was collected before bypass, after bypass, and daily after surgery and serum cardiac troponin I level (micrograms per litre). The demographic data, cardiac diagnoses, types of surgery performed, and peri-operative parameters were collected.
ResultsOf the 21 children enrolled consecutively, five were infants. Among the 21 patients, four patients had post-operative peak troponin values greater than 100 (three were infants) and all four patients survived and had normal left ventricular systolic function upon discharge echocardiogram. The five infants had peak troponin levels of 222.3, 202, 129, 26.7, and 82.3. The post-operative peak troponin levels were significantly higher in infants (mean 132.5 with a standard deviation of 81.6) than in the older children (mean 40.3 with a standard deviation of 33.4), although there was no significant difference in bypass time, bypass temperature, cross-clamp time, or the length of stay in the intensive care unit between the two age groups.
ConclusionsHigher troponin release is seen in infants in comparison with older children after bypass for similar surgeries. A troponin level greater than 100 after bypass does not necessarily predict death or a severe cardiovascular event in the very young.

Bryan Caplan's book could only have been written by an economist. This has a good side and a bad side. On the good side, Caplan makes an important contribution to the public-opinion literature by analyzing the American people's economic views—an area overlooked by most previous researchers. And he asks a crucial question: Are the public's economic views wrong?

The coronary arteries, the vessels through which both substrate and oxygen are provided to the cardiac muscle, normally arise from paired stems, right and left, each arising from a separate and distinct sinus of the aortic valve. The right coronary artery runs through the right atrioventricular groove, terminating in the majority of instances in the inferior interventricular groove. The main stem of the left coronary artery bifurcates into the anterior descending, or interventricular, and the circumflex branches. Origin of the anterior descending and circumflex arteries from separate orifices from the left sinus of Valsalva occurs in about 1% of the population, while it is also frequent to find the infundibular artery arising as a separate branch from the right sinus of Valsalva.
Anomalies of the coronary arteries can result from rudimentary persistence of an embryologic coronary arterial structure, failure of normal development or normal atrophy as part of development, or misplacement of connection of a an otherwise normal coronary artery. Anomalies, therefore, can be summarized in terms of abnormal origin or course, abnormal number of coronary arteries, lack of patency of the orifice of coronary artery, or abnormal connections of the arteries.
Anomalous origin of the left coronary artery from the pulmonary trunk occurs with an incidence of approximately 1 in 300,000 children. The degree of left ventricular dysfunction produced likely relates to the development of collateral vessels that arise from the right coronary artery, and provide flow into the left system. Anomalous origin of either the right or the left coronary artery from the opposite sinus of Valsalva can be relatively innocuous, but if the anomalous artery takes an interarterial course between the pulmonary trunk and the aorta, this can underlie sudden death, almost invariably during or immediately following strenuous exercise or competitive sporting events. Distal anomalies of the coronary arteries most commonly involve abnormal connections, or fistulas, between the right or left coronary arterial systems and a chamber or vessel.
We discuss the current techniques available for imaging these various lesions, along with their functional assessment, concluding with a summary of current strategies for management.

By using Cross Spectral Density processing between high time resolution radio intensity observations of pulsars made simultaneously at two different receiver frequencies, several high frequency periodicities that are persistent over many pulses were discovered. A possible interpretation in terms of neutron star vibration models is discussed.

We have examined approximately 550,000 pulses from PSR 0531+21 and have detected 1814 giant pulses 6σ or more above the background noise level. We hae measured the relative arrival times and the energies of both the main pulse and interpulse, and have found that the giant pulses arrive somewhat earlier than the peaks in the average profile. Approximately 7% of the main pulse and 1% of the interpulse energy is in the form of giant pulses 8σ more above the background level. No giant pulses have been detected in the precursor at the 8σ level. In addition we have found a slight, although apparently real, rise in emission in the region where an interpulse precursor would be if the emission is symmetric.
